To provide supplemental assistance to community-based shelters including SpNS as needed, and to assist with discharge planning activities for elderly and/or disabled persons housed in these shelters to ensure a safe transition from the shelter environment to home and/or relevant residential facility.

• Sorting the placement options post disaster for elders.
• Educating the executives, directors/management of Assisted Living Facilities, skilled nursing, etc. about our Sarasota COAD and Strike Team
• Educating shelter staff of the specific needs of the elder population.
